  • Program Overview

    Dear Families,

     

        HLPUSD’s Adventures After Class After School program provides an important educational and developmental setting for an increasing number of children and youth. These programs are free and offer opportunities for students to learn, develop, explore, and have fun beyond the normal school day. HLPUSD’s After School Program is a state program funded by the 2002 voter approved, Proposition 49 (ASES) and AB 181, AB 185 and SB 114 (ELOP).  After School Education and Safety (ASES) state guidelines require regular attendance.  Students with 3 consecutive unexcused absences per month can be dismissed from the program and given to another child who is on the waitlist.  Documentation will be required for “excused absences” including medical appointments, and special circumstance approved in advance by the After School Program Site Director.    

       The programs run from the end of the regular school day until 6:00 PM, Monday through Friday. To ensure students receive the full benefits of the program, we encourage families to keep their children enrolled until at least 5:00 PM (Early Release) each day. An Early Release option at 4:00 PM is available for families with TK/Kinder students. If your child is picked up prior to Early Release regularly they can be dismissed from the program. There is a list of students who are waiting to participate in the HLPUSD's After School Program.

        Our goal at Hacienda La Puente Unified School District is to provide students with academic enrichment opportunities, physical and other activities designed to complement their regular academic program. We also aim to assist with literacy and related educational development services for the families of our students, all in a safe, clean environment.

  • Please read the following information carefully. In order for a student to be enrolled in Hacienda La Puente Unified School District’s After School Program, this form must be signed indicating understanding and consent.

     

    BASIC INFORMATION AND RULES

    Enrollment:

    • Space is limited and priority will be given to students who attend the program in its entirety until 6pm.
    • Early enrollment will be given to students currently enrolled in the program who have met all program requirements and are in good standing (good attendance, good behavior etc.) Remaining open slots will be on a first come first serve basis.
    • After School Program enrollment will vary depending on site. If the After School Program is at full, a waiting list will be generated.
    • Completed applications will require a minimum of 3 working days to be processed to determine availability and program placement.

     Attendance:

    •  After School Education and Safety (ASES) state guidelines require regular attendance.  Students with 3 consecutive unexcused absences per month can be dismissed from the program and given to another child who is on the waitlist.
    • Documentation will be required for “excused absences” including medical appointments, and special circumstance approved in advance by the After School Program Site Director.
    • Students picked up prior to 5:00 pm (Early Release) regularly can be dismissed from the program.
    • An Early Release option at 4:00 PM is available for families with TK/Kinder students.
    • HLPUSD After School Program staff are NOT allowed to sign-in/sign-out students.
    • Departure prior to end of the program will require early release documentation
    • Students are not allowed to walk home without written parent/guardian approval and addition to “Walker Authorization List” (option only available for Middle School students.)
      •  Students on the “Walker Authorization List” must walk home immediately after sign-out.
      •  Walker Authorization form must be on-file with the HLPUSD After School Program.
    • Students will not be allowed in program if they do not report immediately to the program after the regular school day ends (unless proper late arrival policy has been submitted.)

     General Daily Schedule:

    • Program check-in (time varies by site)
    • Homework support until 3:00 pm
    • Supper, 3:00 pm - 3:30 pm
    • Enrichment Program, 3:30 pm – 5:30 pm (i.e. Arts, Assemblies, Games, Leadership, Middle School DJ Club, Sports, STEM, tutoring, etc.)
    • Program closes at 6:00 p.m.
    • Specific times for check-in, enrichment programs and activities will vary with each site. Please ask your Site Director for your site’s hours and daily schedule.

    Student Pick-Up:

    • Students can be picked-up from designated program check-out till 6:00pm.
    • Student safety is our priority; a current California ID or California driver's license may be requested, and the adult must be 18 and over.
    • Only the parents or guardians listed in the student's AERIES account are permitted to pick them up from the program. It is the parent's responsibility to ensure their AERIES contact information is current with the school office.
    • Students picked up after dismissal time (6:00 pm) might be cared for by local law enforcement.
    • Late pick-ups could be cause for dismissal.
    • Student’s may not be released to parent/guardian if they appear to be under the influence or intoxicated.     
    • Verbal authorization will NOT be permitted by parent/guardian for student(s) to be released to a non-authorized designee.

     Emergency Contact Information:

    • It is important that parents/guardians give UPDATED emergency information whenever it changes to the After School Site Director.
      • Please make sure your information MATCHES Hacienda La Puente Unified School Districts student information system (AERIES). Changes in information must be provided to the School AND After School Program Site Director.
  • Discipline:

    • Program rules must be followed at all times in addition to Hacienda La Puente Unified School Districts standards and rules.
    • The After School Program is a privilege!
    • Students must remain in their designated tables, areas and only handle their own items unless given permission otherwise.
    • Leaving supervised group and/or classroom without permission may result in immediate dismissal from the program.
    • Disruptive or disrespectful behavior toward other students or staff may be reason for dismissal.
    • Progress reports, grade checks and behavior will determine a student’s eligibility to participate in team activities and/or competitions. Students must maintain a 2.0 GPA.
    • Destroying or defacing school or personal property may be grounds for dismissal from program and parent/guardian will be responsible for the cost of damage.
    • Electronics (Cell phones, iPad, headphones, etc.) are not allowed during program hours. If lost or misplaced, the After School Program and Hacienda La Puente Unified School District will NOT be held responsible.
      • Recording, taking video or capturing photos of oneself or others is strictly prohibited during program.
      • Any violation of this policy may be cause for program dismissal!
    • Any act of violence (hitting, pushing, kicking, harassment, bullying, etc.) or infractions towards another student or staff could result in automatic removal from the program.
       

    COVID 19 INFORMATION

     

    HLPUSD follows the Los Angeles Department of Public Health guidelines which are subject to change based on the Los Angeles Department of Public Health guidelines.     AFTER SCHOOL PROGRAM ELEMENTS

     

    • Education and Literacy Element- An educational and literacy element provides tutoring and/or homework assistance designed to help students meet state standards in one or more of the following core academic subjects: reading/language arts, mathematics, history and social studies, or science. A broad range of activities may be implemented based on local student needs and interests. 
      • Homework Assistance:
        • One hour will be designated for homework assistance.
        • Intervention may be provided as additional homework assistance.
        • Homework completion during program is NOT guaranteed.
          • It is the parent’s/guardian’s responsibility to review and ensure their child’s homework is completed at home.
        • Students are responsible for bringing materials or devices needed for assignment completion.

     

    • Educational Enrichment Element- The educational enrichment element offers an array of additional services, programs, and activities that reinforce and complement the school’s academic program. Educational enrichment may include, but is not limited to, positive youth development strategies, recreation and prevention activities. Such activities might include visual and performing arts, music, physical activity, health/nutrition promotion, and general recreation; career awareness and work preparation activities, also community service-learning; and other youth development activities based on student needs and interests.  Enrichment activities may be designed to enhance the core curriculum.

     

    • Nutritious Supper- the snack supper component of the program lasts 30 minutes. The purpose is to provide students with a nutritious supper while giving them a chance to interact with their peers. This helps them focus during the rest of their time in the after school program.

     

    Parental/Guardian Support:

    • Parent/guardian are welcomed and encouraged to discuss their child’s behavior with the Site Director.
    • Being civil in discussing issues is imperative. If civility is not followed, placement in program could be affected.
    • Any current custody issues, restraining orders and related documentation, must be submitted, and disclosed to the Site Director.
    • Joint custody of separated parents/guardian will require both parties to complete registration forms.
    • Parent/guardian visitation while the afterschool program is operating is not permitted without prior written approval.
    • Any change to medical or contact information must be immediately updated with the Site Director.
    • Parents/guardians are encouraged to attend and participate in events, activities, and other special projects such as;
      • Parent Education Workshops- reinforce knowledge and tools to support your student(s) success.
      • Parent University.
      • Back-to-School Nights.
      • Parent Teacher conference and/or School Open House.

  • In case of an emergency and I cannot be reached,   *  authorize, pursuant to the provisions of Section 258 of the California Code of Civil Procedure, any and all attention as may be deemed necessary by the physician/medical advisor in charge. I also realize that local law enforcement and paramedics may be called in certain circumstances in order to ensure emergency treatment. I understand and abide by the stated terms of this release.   • In consideration of being allowed to participate in any capacity in the After School Program in the Hacienda La Puente Unified School District athletics/sports program, and related events and activities, the undersigned:

    1.     Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.

    2.     Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. 

    3.     Assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent total disability or death.

    4.     Release, waive, discharge and covenant not to sue HLPUSD, Champions, Del Haven Community Center or Boys & Girls Club of West San Gabriel Valley & Eastside and its affiliated clubs, their respective administrators, directors, agents and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessons or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in while or in part by the negligence of the releases or otherwise.

  • The Hacienda La Puente Unified School District’s After School Program takes place each school day from dismissal until 6:00 PM. Students participating in the program are highly encouraged to attend the program Monday-Friday until 5:00 PM (Early Release) to gain the most benefit from the program and to be academically and socially successful. An Early Release option at 4:00 PM is available for families with TK/Kinder students.

    In the event the student must arrive late or leave early from program, we ask “authorized adults” to write their qualifying code for early release/late arrival in the appropriate column on the student’s sign-out sheet (see early release codes below):

  • The Early Release Policy is not intended for the daily early departure (prior to 5:00pm) of students.  Families that regularly use early release may result in the student being dismissed from the program.  Early release should be used only when necessary, including medical appointments and emergencies.  Please inform Site Directors in advance when these circumstances arise.

    Students with multiple early release days can be dismissed from the program and given to another student.  There is a list of students who are waiting to be enrolled into HLPUSD After School Program.

    To authorize your middle school student to walk home from the Hacienda La Puente Unified School District’s After School Program, this form must be signed and returned to the Site Director. Your child will not be allowed to leave campus unaccompanied unless and until this form has been received by HLPUSD’s After School Program Site Director.

I am aware that once my child leaves the school campus, there is no adult supervision for them provided by the Hacienda La Puente Unified School District's after School Program staff.     • If you wish to have your child released prior to the end of program, which is 6:00pm, Parent/Guardian must call or text the Site Director.   
      • Calls or texts to your student to relay the message to the site director will NOT be accepted.
    • You will assist HLPUSD’s After School Program staff in ensuring your child’s safety by adhering to the guidelines outlined above
    • You agree that ANY CHANGES from the above permission will be communicated (in writing) to the HLPUSD’s After School Program Site Director prior to changes being made.
    • This authorization form will remain in effect unless and until you have notified the HLPUSD’s After School Program in writing that it has been rescinded.

    If you have submitted your registration form between May 13 - July 19th you will be contacted by your After School vendor by July 26th.

    All students whose registration forms are received after July 19th will be placed on a waitlist and your After School vendor will contact you as soon as a spot becomes available.
